Health and Environment Survey
To share your knowledge about cancer and the environment in your area confidentially, please
complete our survey following the link below. Please complete the information for yourself, this will include the opportunity to report on your family history with cancer. This takes the average respondent 10 minutes, but you can spend longer if you have a lot to report. You can also leave and return to the survey at any time.
All data are confidential and secure.
Survey explained
To learn more, people over the age of 18 can join our email list. You will then be invited to complete optional and confidential surveys that help us gather data about the health and environmental concerns in Grundy County! You can complete these yourself and forward to family and neighbors.
